I travelled yesterday. The form was distributed to everyone on board. It's a yellow card with lots of detailed info to fill in! I was wondering where and why do they wish to keep all that information.
I got the answer soon: nobody cared to collect cards before or after immigration. Mine went directly in the garbage bin well before to reach the hotel. Indonesia boleh.
